Office Policies & Procedures
Office Hours/Telephone Calls
We see patients by appointment only to ensure that sufficient time is allotted for each patient’s care. We believe in the value of your time, and will do our best to keep you from waiting. We would appreciate a 24-hour notice if you find it necessary to cancel your appointment. Please note that any “NO SHOW” appointments will not be rescheduled and will be charged $25.00. Our telephones are answered from 9:00am until 4:00pm. Our employees have been instructed to handle all incoming calls. This allows Dr. Weyrich time to attend to scheduled patients with minimal interruptions. If your call cannot be handled by the office staff, your message will be forwarded to the doctor and he will return your call at his earliest convenience.
Prescriptions and Refills
Prescription refills will only be handled during regular business hours, and only if you are currently under our care. For prescription refill requests you must have the name and telephone number of your pharmacy handy when you call or you may have your pharmacy fax a refill request to our office to 724-942-4075. Please bring a current medication list with every visit.
Referrals
Managed care patients are responsible to obtain a referral for all visits at our office, if necessary. Please contact your primary care physician as soon as possible to assure your referral has been completed. If you come to the office without a referral, you may be asked to reschedule.
Release of Medical Records
Release of any medical records must be authorized in writing. Please fill out our Authorization For Release Of Medical Record Information form and mail or fax to us. There is a fee of $25 for the retrivial of the records.
Copays & Payments
We accept cash, and checks only for all copays. Any returned check will be assessed a charge for bank fees. All accounts over 30 days old will be subject to a $25.00 late fee. All "No Show" appointments will be assessed a $50 no-show fee.
